Many patients have requested we ease the paperwork burden when they visit our office.

Therefore we have posted our New Patient and Optos Consent forms both in Word and pdf* formats which you can either fill-in electronically and submit by email, or printout and bring to your appointment. These forms will help save you time and ensure our records are correct. Consider this yet another patient satisfaction tool!
